Question Customer Cancel Account

Anybody have a suggestion for the best way to have a customer cancel an account? I see in HSphere that a customer can click on Money Back within the 30 day period. What method should they use after that. Am I not seeing a "Cancel Your Account" link? Should they just open a Help Ticket from the Control Panel and request? Need for my FAQ page.

Any suggestions would be greatly appreciated.
(unless I'm just blind, then it's more of a 'duh!' than a suggestion)

Thanks.

We use a very simple form that goes right to our billing department and it as worked fine for years. Take a look:
